Description: CKM health involves the optimization of several interrelated systems, including adiposity, endocrine and renal systems. This session will include a case presentation to highlight the potential barriers to optimal CKM health followed by presentations targeting pathways to promote health across systems. Specifically, we will learn about pharmacoequity and access to costly therapies and access to specialty care for services. The session will also include a discussion on care models to address the need for interdisciplinary care. Additionally, we will hear from a primary care physician regarding their approach to optimizing CKM health. Finally, there will be a multidisciplinary discussion that brings back the lessons from each presentation to the case.
